SamEng: Translation of mRNA is a prerequisite for cell proliferation,
differentiation and viability. We have studied the effect of ribosome
protein factors (GPRE) on acute myeloid leukemia (AML) blast cells.
Ribosomes were isolated from MPC-11 cells using ultra-centrifugation.
GPRE were extracted using a high KCl procedure. Blast cells from six
AML patients were grownin suspension cultures for 24 and 96 h. GPRE
or granulocyte macrophage-colony stimulating factor (GM-CSF) were
added at the start of the incubation. GPRE, but not GM-CSF, prevented
chromatin condensation and fragmentation of blast cell nuclei in
AML-M2, -M4 and -M5 and the loss of nucleoli in AML-M2 and -M5. The
fraction of phagocytosing blast cells in AML-M1, -M2, -M4 and -M5 was
increased by GPRE. GPRE stimulated opsonin-dependent and -independent
attachment and internalisation of N. meningitidis. GPRE increased the
fraction of blasts expressing CD11b and CD32 in AML-M2 and -M5. GPRE
diminished the fraction of AML-M5 cells bearing CD35 and CD32. GPRE
also decreased the fraction of CD11c-bearing AML-M2 and -M5 cells.
GM-CSF potentiated effects of GPRE in AML-M1, -M2, -M4 and -M5. GPRE
and GM-CSF in combination affected phagocytosis and surface antigen
expression in blast cells that were not influenced by either factor
alone. Neither GPRE nor GM-CSF induced terminal differentiation or
DNA-synthesis. We conclude that GPRE affects AML blast cell
morphology, function and surface molecule expression, possibly by
inhibiting apoptosis. The effects of GPRE may be mediated by
ribosomal proteins that regulate translation and modulate the
subcellular distribution of mRNA species.

SamEng: Large granular lymphocyte leukaemia (LGL leukaemia) is a rare,
chronic lymphoproliferative bone marrow disease which can be
considered a subtype of chronic T-cell lymphocytic leuka emia
(T-CLL). We describe three patients with large granular lymphocyte
leukaemia. They all suffered from chronic disease with neutropenia
and relative lymphocytosis. An expansion ofmature lymphocytes with
the CD3+, CD8+, CD57+ immunophenotype was demonstrated in all three
patients. Two patients had a history of recurring infections.
Serological findings compatible with rheumatic disease were present
in all three patients, and two suffered from rheumatoid arthritis. We
have made a brief survey of this disease, which is characterized by
relative or absolute lymphocytosis,neutropenia, and an increased risk
of infection. Rheumatoid arthritis and other associated autoimmune
manifestations occur frequently. Large granular lymphocyte leukaemia
should be considered a possible diagnosis in patients with chronic
neutropenia and relative lymphocytosis, in particular if rheumatic
manifestations are also present. Flow cytometric immunophenotyping is
a sensitive method in diagnosing this disease.

SamEng: T lymphocytes are important both for the host defence against
infections and probably also as antileukaemic effector cells in
patients with acute leukaemia. To investigate the T lymphocyte
cytokine repertoire of clonogenic T lymphocytes, CD4+ and CD8+ T
lymphocyte clones were prepared from acute leukaemia patients with
chemotherapy-induced cytopenia (leucocytes <0.5x 10(9)/l). A majority
of both CD4+ and CD8+ clones secreted detectable interleukin-2
(IL-2), IL-10, IL-13,granulocyte/macrophage-colony-stimulating factor
and interferon gamma (IFNgamma) in response to phytohaemagglutinin +
accessory cells (Epstein-Barr-virus-transformed B cell line,
80-Gy-irradiated). The CD4+ clones showed significantly higher levels
of IL-10 secretion than the CD8+ clones. Decreased levels of IL-2,
IL-13 and IFNgamma were observed when acute myelogenous leukaemia
(AML) blasts were used instead of cells from the B cell line as
accessory cells during phytohaemagglutinin activation, but the
differences in IL-13 and IFNgamma levels were reversed by addition of
exogenous IL-2. On the basis of these results we conclude: (i) the
remaining clonogenic T lymphocytes derived from acute leukaemia
patients with therapy-induced leucopenia can respond to activation
with a broad cytokine response, and T-cell-derived cytokines may then
contribute to cytokine responses during complicating infections in
these patients; (ii) although T cells can modulate AML blast
functions a nd mediate antileukaemic effects, the leukaemia blasts
will also modulate T cell functions and alter the cytokine profile of
activated T lymphocytes.

During the period 1990-1995 2.5 specialists in haematology qualified
per year. In to meet future requirements for haematologists at
Norwegian municipal and university hospitals, it has been estimated
by the Norwegian Society for Haematology that the number of
specialists qualif ying per year should be increased to ten for the
next ten years.

SamEng: Cytokines are important regulators of acute myelogenous leukemia
(AML) blast proliferation. For a subset of patients the AML blasts
show constituti cytokine secretion and can undergo autonomous
proliferation in vitro, whereas for other patients the blasts are
dependent on exogenous cytokines for proliferation. The capability of
autocrine proliferation is an adverse prognostic factor in AML. The
three cytokines interleukin (IL)-4, IL-10 and IL-13 modulate in vitro
blast proliferation, but the final effect of each cytokine
(enhancement/inhibition/no effect) depends on differences between
individual patients as well as the presence of other exogenous
cytokines. In contrast to these divergent effects on blast
proliferation, all three cytokines inhibit constitutive in vitro
cytokine secretion by AML blasts. Because of the divergent effects on
blast proliferation, it seems less likely that clinical therapy with
these cytokines can be used to directly modulate AML blast
proliferation. However, their effects on normal immunocompetent cells
(and possibly the antigen-presenting capacity of AML blasts) are
easier to predict. Thus direct (therapy with exogenous IL-4, IL-10 or
IL-13) or indirect (enhancement of endogenous release of IL-4, IL-10
or IL-13) modulation of th ese cytokine effects on immunocompetent
cells may become a useful clinical approach for enhancement of
antileukemic immune effects. Such a modulation of immune reactivity
can be used either as in vivo patient therapy or as manipulation of
stem cell grafts prior to transplantation.

severe chemotherapy-induced leukopenia. We used a limiting dilution
assay to investigate whether remaining CD4+ and CD8+ T lympocytes
derived from such leukopenic patients could be activated and undergo
clonogenic proliferation. The activation signal in our model was
accessory cells (irradiated normal peripheral blood mononuclear
cells) + phytohaemagglutinin (PHA)+ interleukin-2 (IL-2). During
severe leukopenia a majority of circulating lymphocytes were CD4+ T
cells. Clonogenic proliferating T lymphocytes were detected for all
patients. Higher frequencies of clonogenic cells were dletected in
the CD8+ subset as compared to the CD4+ subset. However,for both
subsets frequencies of proliferating cells were decreased compared
with healthy individuals. The CD4+ and CD8+ lymphocytes were also
capable of proliferation in response to alloactivation, and accessory
cells mainly containing acute myelogenous leukaemia blast were
efficient as accessory cells for activation. For the CD4+ cells,
increased proliferation was detected in the presence of acute
myelogenous leukaemia (AML) blasts compared with normal accessory
cells. Based on our results we conclude that: 1) although acute
leukaemia patients with therapy-induced leukopenia have both a
quantitative and a qualitative T-cell defect, 2) the remaining T-cell
population includes a subset capable of clonogenic proliferation.

SamEng: Altogether 40 patients aged 13-91 y (average 58 y) with vertebral
osteomyelitis were treated at the Bergen University Hospital between
July 1987 and June 1997. All patients presented with back pain, 33
(83) had vertebral tenderness, and 26 (65) patients were febrile.
The duration of symptoms before diagnosis was < 3 weeks in 13
patients, and from 3 to 16 weeks in the remaining 27 patients.
C-reactive protein (CRP) level and erythrocyte sedimentation rate
(ESR) were elevated in 39 and 38 patients, respectively.
Staphylococcus aureus was the most frequent cause of osteomyelitis
followed by Streptococcus spp., Escherichia coli and Mycobacterium
tuberculosis. Magnetic resonance imaging was superior to other
radiological methods and demonstrated changes consistent with
osteomyelitis in all 23 patients examined with this method. 35
patients survived. 18/35 surviving patients had pareses and 17
underwent surgery with drainage of abscesses or laminectomy. All 35
patients made a good recovery and only 3 patients experienced
permanent pareses. The diagnosis of vertebral osteomyelitis is easily
missed, and treatment is often delayed, particularly in the elderly
in whom signs of sepsis may not manifest. However, persisting
localized pain and tenderness over the spine together with elevated
CRP and ESR should prompt the physician to consider vertebral
osteomyelitis. Fever and leukocytosis may support the diagnosis, but
may not always be present.

but significant increase in the percentages of CD15+,CD64+ cells with
sustained CD34+ cell proliferation. We conclude that IGF-1 can
enhance the in vitro proliferation of committed progenitor cells
derived from umbilical cord CD34+ cell s.

SamEng: A one-step flow cytometric (FCM) assay has been developed to
quantifyboth opsonin- and antigen-dependent phagocytosis and
intraphagocyte oxidative burst responses. Meningococcal outer
membrane structures (OMV) were adsorbed to fluorescent polystyrene
beads, opsonized with serum, and exposed to leukocytes. FCM
parameters of phagocytosis wereevaluated in combinations with
oxidative burst indicators. Rhodamine-123 was the most sensitive
indicator and was compatible with quantitation of phagocytosis. The
phagocytosis and oxidative burst responses induced by OMV beads were
dependent on both antigens and opsonins. Increased human opsonic
responses against OMV were induced during clinical meningococcal d
isease. A dissociation was noted between phagocytosis and oxidative
burst in individual cells, indicating that functional opsonins
against OMV components may differin their ability to stimulate
phagocytosis and oxidative burst responses. The method facilitates
evaluation of purified bacterial structures as mediators of
opsonin-dependent phagocytosis an d intracellular oxidative microb
icidal mechanisms, which is of interest in the complex process of
selecting bacterial antigens as constituents of certain vaccines.

SamEng: We found a higher plasma concentration of total homocysteine (tHcy),
an independent risk factor for cardiovascular disease, in patients wi
th hypothyroidism (mean, 16.3 
mol/L; 95% confidence interval [Cl],
14.7 to 17.9 mol/L) than in healthy controls (mean, 10.5 mol/L: 9
5% CI, 10.1 to 10.9 mu mol/L). The tHcy level of hyperthyroid pa tien
ts did not differ significantly from that of the controls. Serum crea
tinine was higher in hypothyroid patients and lower in hyperthyroid p
atients than in controls, whereas serum folate was higher in hyperthy
roid patients compared with the two other groups. In multivariate ana
lysis, these differences did not explain the higher tHcy concentratio
n in hypothyroidism. We confirmed the observation of elevated serum c
holesterol in hypothyroidism, which together with the hyperhomocystei
nemia may contribute to an accelerated atherogenesis in these patient
s.

orbed. Thus, a formulation with OMVs and C-ps represents a safe and h
ighly immunogenic vaccine, even without alumini um hydroxide.

SamNor: POEMS-syndromet er en sjelden multiorgansykdom hvor hovedsymptomene
er polyneuropati, organmegali, endokrinopati, monoklonal gammopati og
hudforandringer. Gammopatien er ofte betinget i et myelom eller et
plasmocytom som kan påvises radiologisk og histologisk. Patogenesen
er uklar, men sannsynligvis immunbetinget. Utviklingen av syndromet
kan skje over flere år. Primært må den hematologiske grunnsykdom
behandles. I en del tilfeller kan dette stabilisere utviklingen av
tilstanden. Det er viktig å kjenne til tilstanden for å kunne se
sammenhe ngen mellom de ulike symptomene og sikre at riktig
behandling blir igangsatt. Vi presenterer tre pasienter med
POEMS-syndromet og en gjennomgang av litteraturen.
SamEng: POEMS syndrome is a rare multisystemic disorder characterized by a
variable constellation of polyneuropathy, organomegaly,
endocrinopathy, monoclonal gammopathy and skin changes. The syndrome
is believed to be immune-mediated and is probably related to the
pathological monoclonal component caused by a myeloma or a
plasmocytoma. The treatment is aimed at the und erlying plasma cell
dyscrasia. We have studied three patients with this rare syndrome.
This article presents the clinical picture of each one and a survey
of the literature.

SamEng: OBJECTIVE: To study mechanisms behind postprandial symptoms in
patients with diabetes mellitus and the effect of nitric oxide (NO)
on gastric accommodation and symptoms in these patients. DESIGN: A
double-blind, placebo-controlled, randomized trial was designed in
20patients with type 1 diabetes (10 male and 10 female, aged 35.3 +/-
7.6 years). METHODS: 0.5 mg sublingual glyceryl trinitrate (GTN), a
donor of exogenous NO, or placebo was administered 5 min prior to a
500 ml soup meal. Gastric accommodation of the meal was assessed by
abdominal ultrasound. Accommodation in proximal stomach was
visualized in a sagittal area (Psa) and a frontal diameter (Pfd) and
accommodation in distal stomach was visualized in a sagittal area of
the antrum (Asa). Symptoms were assessed using visual analogue
scales. RESULTS: Psa correlated significantly (r = 0.57, P = 0.015)
with perception of fullness 5 min after the meal, whereas Pfd
correlated significantly (r = 0.67, P = 0.004) with nausea at 15 and
at 25 min after the meal. Asa correlated (r = 0.50, P = 0.05 ) with
pain at 5 min, 10 min (r = 0 .50, P = 0.05) and 25 min (r = 0.68, P
=0.007). GTN had no significant effect on Psa or Pfd, but reduced
significantly (P = 0.05) Asa (1 3.5+/- 4.5 cm2 with GTN vs 16.1 +/-
4.3 cm2with placebo). GTN increased significantly (P = 0.04) the
intragastric proximal/distal meal distribution ratio
(proximal/distalsagittal area), but had no significant effect on
symptom scores. CONCLUSION: In patients with diabetes, a large
proximal stomach is associated with perception of fullness and a
large antrum is associated with perception of pain after a me al.
Sublingual administration of GTN prior to the meal decreases the
antral area andimproves the intragastric meal distribution, but fails
to improve symptoms.
