SAMMEND: Mens Helsedirektoratets årsmeldinger for Den offentlige
tannhelsetjenesten viser kariesutvikling på landsbasis og i fylkene,
omfatter denne analysen også årsmeldingene for 1985, 1988 og 1991 fra
hvert enkelt tannhelsedistrikt (unntatt Oslo). På nasjonalt nivå
fortsatte den positive trenden for alle indikatorårskullene fra
1985-1991. Antallet fylker hvor kariesprevalensen steg fra det ene
året til det neste økte, spesielt i perioden 1988-1991. Indikasjonene
på en utflating eller økning av kariesprevalensen var sterker på
distrikts- enn på fylkesnivå og tydligere for 5- enn for 12-åringer.
Intervjuer med overtannleger i distrikter med negativ kariestrend fra
1988 til 1991 og i distrikter hvor karies forsatte å synke, avdekket
ingen forklaring på hvorfor karies økte i noen distrikter, men
fortsatte å synke i andre. Altså er det behov for nøye overvåkning av
kariesituasjonen for å se om presenterte funn skal tolkes som
svingninger omkring et utflatningsnivå, eller om de signaliserer
begynnende økning i kariesprevalens hos indikatorårskullene.

SAMMEND: After the Second World War it was realized that shortage of dentists
would become a serious problem in Norway in years to come. To
alliviate the problem students were sent abroas to obtain a dental
qualification. Post-war economic constraints and indecision
concerning the establishment of one larger and two smaller dental
schools meant that Norway's own educational capacity for dentists did
not reach 130 per year until shortly before the decline in caries
prevalence became evident in the early seventies. The labour market
situation became gradually more difficult during the seventies and
early eighties with some unemployment. In 1983 the government reduced
the intake of dental students from 130-105 per year and at about the
same time student drop-out during the dental course started to
increase. The economic boom during the mid-eighties increased the
demand for dental services in the adult population, a development
which allowed more newly qualified dentists to enter private
practice. Delayed decision-making and professional intuition rather
than reational personnel planning based on the use of adequate
epidemiological and socio-dental data in demand or need-based models
have characterized the political process in determining the
educational capacity for den tists in Norway during the last 50
years.

SAMMEND: Pågående og foreslåtte utredninger av behovet for odontologisk
spesialkompetanse aktualiserer også en vurdering av behovet for
lederkompetanse i tannhelsetjenesten. Situasjonsanalysen som
presenteres her, viser at det i forhold til antall lederstillinger i
Den offentlige tannhelsetjenesten mangler 60-70 tannleger som kan
oppfylle Sosialdepartementets krav til kvalifikasjoner for
fylkestannleger og ledere av tannhelsedistrikt. Videre viser det seg
at eksisterende alternativer til den 1-årige videreutdanningen verken
kan fjerne nevnte undersk udd eller dekke fremtidig erstatningsbehov
for ledere.

SAMMEND: Limited attention has been paid to the importance which adults attach
to dental, as opposed to other, life events. Consequently, the
purpose of the present study was to determine how Norwegian adults
rate the need for adjustment to cope with losing teeth or getting
dentures, relative to some other life events, and to study effects of
age, gender, level of education, place of residence, smoking,
drinking and exercise habits. A representative, simple random sample
of 400 residents of the County of Hordaland aged 20-69 yr recieved a
mail questionnaire plus one reminder requesting demographic
information and completion of a 48-item Social Readjustment Rating
Questionnaire (SRRQ) in May 1990. The mean age of the 284 who resp
onded (71%) was 41.1 yr (SD=13.5 yr). The replies to the SRRQ were
given on a graphic rating scale with end points "not difficult at
all" (0 cm) and "more difficult than anything" (20 cm). Mean values
were calculated and used to rank the life events. "Losing one or more
teeth" was ranked 30 (#~x=11.8; SD=5.6) and "getting dentures" was
ranked 33 (#~x=13.8; SD=5.5). "Losing one or more teeth" was on a par
with life events like "trouble with other relatives" (#~x=11.7).
"Getting dentures" occupied a position between "trouble with
wife/husband"(#~x=14.3) and "wife/husband stopping work" (#~x=13.8).
Multiple Classification Analysis revealed significant effects of
education on "losing one or more teeth"; and of age, sex andedu
cation on "getting dentures" (P<0.05). Furthermore, it was found that
these two dental life events required an above average degree of
readjustment according to the Social Readjustm en t Rating
Questionnaire.

SAMMEND: The aim of the present study was to collect information about
primary, secondary, and tertiary reasons that dentists gave for
extracting permanent teeth and to determine whether and how
dentist-associated characteristics might influence the relative
emphasis on clinical diagnosis versus non-disease considerations
given as reasons. A national random sample of Norwegian dentists
(n=500) recorded reasons for tooth extraction during a period of 2
weeks in 1988 (respons rate, 70%). Nine hundred and eighty-five
teeth were extracted from 692 patients. Disease-/condition-related
diagnoses topped the list for primary and secondary reasons for
extraction, whereas patients' wishes, economy, and esthetics came in
the forefront among tertiary reasons. Logistic regression analysis
showed that the choice between clinical diagnosis and non-disease
considerations as primary and secondary reasons for extraction wass
ignificantly but moderately influenced by variables associated with
the dentist.

SAMMEND: The aim of the present study was to validate dentists' periodontal
reasons for extraction by comparison with the in vitro periodontal
status of extracted teeth. A national systematic random sample of
Norwegian dentists (n=500) was requested to record primary, secondary
and tertiary reasons for tooth extraction for a period of 2 weeks in
1988. The response ratewas 70%. The extracted permanent teeth from
the first to patients of each dentist were collected. Of the 365
teeth, 329 satisfied the criteria for assessment of periodontal
attachment after staining. Using a dissecting microscope (10 x), 4 to
8 linear measurements were recorded per tooth. 159 of the 329 teeth
had loss of periodontal support. Of the 93 teeth for which the
dentists' reason for extraction included periodontal considerations,
1% had 1-10% loss of attachment, 59% had 11-50% loss and 40% had
51-76% loss of periodontal support. There was a significant
correlation between in vitro measurements of attachment loss and a
ranking of teeth on a scale from 1 to 3 based on the dentists'
emphasis on periodontal rea sons for extraction (The Spearman
Rs=0.29, p<0.01). The results suggest that the forceps level for
removal of teeth for periodontal reasons was set at a relatively
early stage of the disease process by Norwegian dentists, and that
there was a weak association between attachment loss and the
dentists' emphasis on periodontal reasons for extraction.

SAMMEND: The aim of the present study was to investigate the relationship
between linear and area measurements of periodontal attachment loss,
and to determine the root surface area on different tooth-types.
Extracted permanent teeth were collected in a national survey
including a systematic random sample of 500 Norwegian dentists. The
response rate was 70%. Alltogether 329 teeth were recieved, 81 of
them (excluding 3rd molars) were subjected to both linear and area
measurements of attachment loss. Using a dissecting microscope (10
x), 4 to 8 duplicate linear measurements were recorded per tooth. The
areas of both lost and remaining periodontal attachment were measured
by a method based on controlled release of Cu and Co metali ons from
coatings applied to the root surface. Linear loss #~x=49.2%. The
corresponding measurements for multi-rooted teeth (n=18) were #~x=
30.3% and #~x=35.1%, respectivily. Thecorrelation between linear
and area loss was r=0.78 for single-rooted and r=0.75 for
multi-rooted teeth (p<0.01). The lower correlation in multi-rooted
teeth is probably due to the more complex root anatomy. It is
concluded that there is a strong correlation between linear loss of
attachment and area of periodontal support lost for teeth with early
to moderate los s of attachment, and that linear measurements tend to
underestimate the remaining periodontal attachment.
